Distracted walking, this practice is rampant!

§ Out of 4,129 pedestrians surveyed, 814 pedestrians (20%) using a portable device while walking. § 252 (31% ) of those distracted pedestrians were involved in a safety-critical incident. Survey by Monash University – October 20, 2022

Actions Taken § Reviewed TSF Risk Assessment Plan, risk associated with this ramp area addressed, no update needed. § 2 Anti-slip strips installed on Mon, Jan 22., 2024 § TSF team members briefed on Tue, Jan 23, 2024, on all precautionary measures . § Thinner anti-slip mat ineffective, additional 4 anti-slip strips installed on Fri, Jan 26, 2024. Recommendations • Do not rush • Ramp area kept dry at all times – everyone responsibility to do so. • Non-slip shoes to be worn at all times.

To add 2 more non-slip strips

Floor slippery when wet, to place anti-slip mat (sourcing as current anti-slip mat impede the opening and closing of doors during operation hours )
